Dear one by with heart is a prepositional phrase; it cannot be separated while another by is used to introduce Agent. There is no problem whether we use it one time or two times. Now look at the following sentence:

He killed saima by suffocating her. (Active voice)
Saima was killed by him by suffocating . (Passive Voice)
in the above sentence, by has been used two times, which is quite correct.
Dear If you feel difficulty with Active and passive voice, then purchase "Active and Passive Voice Made Easy By Efzal Anwar Mufti" It is a very nice book, which will help you a lot.

Yes you can use "by" two times in a sentence if needed.
If sentence is
1. I learn this poem by heart. (Active Voice)
1. This poem is learnt by me by heart (Passive Voice)

Another example could be:

I will submit the documents by tomorrow. (Active Voice)
The documents will be submitted by me by tomorrow. (Passive Voice)

I hope this would help

Regards
