@ Last Island

1. I dont remember the dates on which Matric and Intermediate results were announced. They're not mentioned in mark sheets as well. what to do? should I write only year?

2. Should Annex A,B,C,D and E be left blank?

3. What do I need to write in major subjects of Matric and Intermediate? Science group or Pre-Medical group?? or subjects like Biology, Physic etc

and for matriculation Science group and for inter Pre-Medical group etc.

Hi plz guide me where is the Treasury Number on the Treasury Receipt(downloaded from FPSC website)??. i have submitted the Fee 1000 in NBP.... but there is no TR # on the receipt... Cashier write a number with red pen on the receipt i.e 15 is it that Treasry Receipt #?? plz help me....

Things required are
Original treasury Receipt for Rs.1000
Date
Branch Name
Branch Code
City/district
so not to worry about serial number.Not important. But 15 is serial number.

This treasury number is written when you submit your challan directly in Treasury Office of your city, but as you have submitted your challan in NBP so you have to write the challan number 15 in the form.

i had downloaded the form from fpsc website and got print up to page 7.should the annex a,b,c,d,e be attached wid the form at the time of submission.plz reply as m going to submit my forms today plz reply
