Please let me know that:

How many photos are required, and where to attest them backside or on front side..


How many copies of each document must be photocopied from matric to master as I have these

-Matric+Inter
Marksheets of both
Pass certificates of both
Pacaa certificates of both
Plus I have attached leaving certificate of metric mentioning date of birth

BA+MA
Marksheets of both
Pass Certificates of both
Note: I do not have their degrees, however I have applied for degree and got recipts (Should I submit recopts too?

And what about character certicates, income etc???

And I have submitted chalan form of 2200/=

I have made account and going to apply online? should I?

Please let me know clearly so that I send them today...

In domicile section which areas are including in Punjab (tribal Area). I have my domicile in Punjab, Lahore. Shall i tick the option of Punjab (inculding Islamabad)?

Page numbers missing in the form are 8, 10, and 12. Where are they? Will it create problem?

No it won't.. Just fill the relevant pages and leave others that are not applicable to you..like DPC, Annex A,B,C

Do mention your roll no(ABCD), year(2013) and exam(CE-2013), but in result column fill in "Result Awaited"... that's what I filled in my form and have sent it too...
