I have almost completed my form, just one thing. On the first and last pages there are boxes that need to be filled (on the first page, Receiving stamp, t.r removed for rs, batch no, diary no, roll no. and on the last page-7th- roll no and district). These will be filled once the form reaches FPSC, right? The only thing that I have to do is sign, check the yes/no boxes, and attach the documents, right? If there is anything else, kindly guide so.

Yes all these things on 1st page will be filled in by FPSC and Roll number on last page too but District is missing due to online application system's technical issue. It had happened last time too so fill in your postal address district sign the 1st and 6th page along with date and place(your current city) on 6th page. Leave the rest blanks to FPSC.

Dear All,

I have a query.
we have to mention our matriculation name in the application form,
my name in matric certificate is mentioned as:

Muhammad Awais
Father name: Muhammad Jahanzeb Khan

But in other documents Father name is mention as Muhammad Jahanzeb, so Shall I write as per matriculation or I can write Muhammad Jahanzeb only.

Yes write your name as per Matriculation certificate.

There is just contradiction of Khan, right ?

Its Muhammad Jahanzeb Khan in Matriculation and the same name Muhammad Jahanzeb in other certificates, only Khan is missing, so just write Muhammad Jahanzeb.
