Contact the concerned authority on the number given on NTS website and on IMU website.
They will ask you to submit documents along with written application for the subject. Don't panic just contact them. I hope it will be clear.

We were asked not to mention it in health dept interview. Later on, those who had mentioned it were selected
Anyway, there are so many things koi kehta hai ye batao koi kehta hai wo na batao. Jitne muh utni batain
