Ever university has its own fee structure, so don't know exactly your case. In my University it was 1000.Rs, a degree form (a real simple form, nothing complex in that) you can get that from the secrecy wing of your university easily and Free.

Bank will be probably nearest to your university campus (ask the secrecy or your HOD). Also in our university you had submit your transcript with the degree form and CNIC. SO do take your transcript/DMC with you while applying for degree...! Wish you all the best..!

Interviews had ended on 2nd of Nov... It was good Alhamdulillah

Whats next for you selected ppl?
Well frankly i don't know the next phase my self. Probably they will send a medical/ Police report letter and after that the will send us Joining/Posting letters... Fingers crossed and waiting for the next letter from F.G.E.I........ Any way..... May you also hear the good news soon........Ammin...!
