Here are some basic exercises that can help you:
  1. spend an hour daily in English conversation with someone who is well articulated
  2. pick one English movie on weekly basis and watch it 3-4 times till you start catching on phrases and accent
  3. start thinking in English
  4. write an essay or speech, or take any available content, and read it in front of the mirror out and loud everyday

Practice is the key, my friend.
1) Learn grammar.
2) Watch Youtube videos
3) READ alot of English stuff i.e newspapers, articles, etc
4) Close the door and speak out loud so you can also hear what and how you are saying it. Improve the deficient areas.
5) Record your voice and hear. Compliment yourself on your progress. Yes, Do it. It works.
6) Think in English. Live in English.

Keep working, Never settle.
Its a slow process but you will surely see progress after some effort.

Start using more and more words of English while speaking Urdu. It will help you while speaking English.

Watch movies and documentaries a lot and also try to speak with yourself if there's no one available whom you could have a conversation with. I would like to suggest you to not spend time on learning accent rather spend time on focusing how to speak clear and grammatically correct English. One can't learn accent even if they spent whole life here in Pakistan, so you just need to focus on your speaking skills by speaking a lot and watching as much as possible.
