Lashari, u can get ur prescription from any private practitioner. no biggie. i asked the guy in-charge for our med exams

anyone w/ the directions to the medical centre where v have to report for our med exams would b greatly appreciated. i checked mapquest and google maps. no luck!

There's this tall building of meteorology department (mehekma e mosamiyat) with a big round structure at its top that every rickshaw guy would know about. There's a petrol pump opposite to that right across the street. A hundred meters ahead on the road besides that petrol pump is your building. Apparently, no one in Lahore knows about A.R.Chughtai road.. so this is how you're gonna get there. I also faced difficulty finding the place last year, coming all the way from Multan & all that.. I hope this method helps though.

got it. thnks a lot dude. the map link was very helpful. i think the name of the road has been changed to birdwood road or sth bcoz tht's wat i saw when i typed in the address in google maps. anyways, thnks again

No it is not available at any NBP branch (in Lahore at least)
What you have to do is to get a hold of form 32A....i repeat....form 32A. Its available at some bookstores, however, play it safe and visit Lahore kacheri or those people sitting infront of the Lahore high court. They have all the forms with them.
You need to fill three copies of that form (pretty easy actually).
Take those three forms to any NBP branch and pay the 100 rupees and they will give you one of the copies as keepsakes (with a stamp and signature on it).

As for the other 750 rupees, you need to pay that (in cash) to the medical examination staff/admin on the day of your test.
