Do i need to send original degree or will they accept Provisional certificate of Bachelors ?

Send Provisional certificate of Bachelors, no problem...But you need to submit the degree at later stage....

Plz tell me whether single application is enough for both the serials i.e. serial no. one and two.

No. Two separate applications must be submitted for serial 1 and 2. Serial 2 is for disable persons.

so it mean that a disabled will submit two forms but why ? why it is needed to submit two forms ? if two forms is submited then there will be two roll numbers with one person which one will be writhed then?

It is clearly mentioned in the PMS KPK Advertisement, that is:
"Note:-
1. The candidates applying against Serial No.2 (Disable Quota) must submit separate application if they desire to appear against Serial No.1. Candidates applying against serial No.1 are advised to submit only one application form. They will be considered for all categories of Serial No.1 after qualifying written portion of competitive examinations. "

dear prestigious i have read all adv but sir i can not get this about submission of two separate forms for disabled why two forms? b/c if we submit 2 form then we will have 2 roll numbers ??? then which one will be written ? iam confused about this....

Just follow the instructions and submit two forms, everything will be clear on later stage. Don't worry for 2 roll numbers issue etc.
