We have to get it from Distt. govt hospital but for that too hard copy of letter from fpsc is required which is still not received. I'm also waiting for that.

absolutely true now a days who is not proficient in MS office especially when he has 16 year education as required for this post.

Brother, I went to STH, met PA to MS, told him that I need such certificate. Upon enquiry the MS told his PA that it was not his job and referred me to a medical specialist in the Medical Ward. There, the specialist upon seeing FPSC notice said, "my name is not mentioned in it, it's just saying the competent authority." So he also refused to sign any certificate. Then I came back to the PA to MS, he took me to Deputy Medical Superintendent. He wrote my chest and height measurement on OPD receipt and was about to sign it when I asked him if he would sign a printout. The DMS agreed, and I went to a composing shop typed a certificate and brought it before him for signature.
It says it's to certify that Mr. X son of Y bearing NIC # has chest measurement of #######.
His height is #####.

I don't know if it's the right format but I'm sending this to FPSC.
Sign and stamp Deputy Medical Superintendent STH.

Believe me it's the worst paper ever used. Some grade 0000 paper I guess. If you believe you need the notification, just print the first page of that PDF file which can be downloaded from FPSC site. Just to convince them why you need it.

sorry to announce this news that the result of Inspector Customs (intelligence) officer is 'disturbed' by the writ of Customs department clerical staff..
i personally visited the FPSC H/Q today to know about the Private institutions' Computer diploma status in "Preventive Officer case" (which is acceptable)..

and the result of Appraiser will be announced 10-15 days (according to Information desk Staff i;e room 2 in H/q)

Brother, it is not about the quality of paper. I know it is worst. My main concern was if there is Roll no. written and it is under signed as a proof that we have to get this medical certificate.

Brother, it means 2 months MS office proficiency certificate from any private institution will serve the purpose. Right?

In that case, FPSC doesn't address you by name, it's Dear Candidate. Get a copy of Page 1 of the notification, the page where your roll number is (highlight your name) and the last page if you must, to indicate the issuing authority. By the way they don't seem that they care even if chairman FPSC issues such a notification.
