Please note the following points that may be relevant to you:

1) First paste 01 photograph on ISSB form then attest from 17 grade or above officer (He should not be Councellor,Nazim,Retired Officer or any of your own relative), such that half of the stamp is on photograph and half on the form, and the remaining 3 should be attested on the back (As said by basit you may attest 5 photographs in case they may require more)

2) In Column 5 count your age up to 14 Nov-12 (or the date mentioned on form)

3) In column 18 "Next of kin" write the name of your father or mother or guardian (where applicable)

4) Please note that all documents should be attested by the same person who attested your photographs.

5) Do not Fill no____ and dated _____ at page 11 only fill your name & address.

6) Don`t write anything in column 6,7,8,31 & 32.

7) Documents that are required to be submitted with the form are:

a) Matric certificate and Marksheets (2 photocopies)
b) Intermediate Certificate/Marksheet (2 photocopies)
c) Self & father`s CNIC (2 copies each)
d) 100 Ruppee Postal order (Blank) from any post office
e) FSc Marksheet/Certificate (2 copies)
f) A-level certificate and its equivalency certificate (2 photocopies)
g) O-level certificate and its equivalency certificate (2 photocopies)
h) Recommended Letter of ISSB

Note: ALL DOCUMENTS TO BE ATTESTED FROM GRADE 17 OR ABOVE OFFICER AND BRING ALL ABOVEMENTIONED ORIGINAL DOCUMENTS AT THE TIME OF SUBMISSION OF FORM 492-S.

mere khayal se scene ye ha ke ye woi form ha jo GDP ki induction k time unhe ISSB k bad dia jata ha once they are through that 5 day test... us waqt unko ISSB recommendation letter issue hota ha..ab humara tou ISSB hota nai ha so ye letter humpe apply nai hota..

But i think we do not have any issb letter?
